What is ClinicalConnect?
ClinicalConnect is a secure online portal that provides health care professionals, such as doctors, nurses, and pharmacists, with real-time access to their patients' electronic medical information (electronic health records) from local hospitals and Community Care Access Centres.

Who uses ClinicalConnect?
• More than 2,500 doctors and other health care providers have access and 28 hospitals across the Waterloo Wellington (WW) and Hamilton Niagara Haldimand Brant (HNHB) LHINs are connected through ClinicalConnect, representing over two million patient records.
• In the WW LHIN, six hospitals, the WW Community Care Access Centre and eight Family Health Teams have access to ClinicalConnect. The hospitals include: Cambridge Memorial Hospital, Groves Memorial Community Hospital, Guelph General Hospital, Louise Marshall Hospital, Palmerston and District Hospital, and St. Mary’s General Hospital). Work is in progress to add data from Grand River Hospital and the Grand River Regional Cancer Centre. |
What information is available on ClinicalConnect?
Through ClinicalConnect, doctors and other health care professionals have access to their patients’ medical information (electronic health records or EHRs) from local hospitals and Community Care Access Centres and cancer centre, including: when they were admitted to hospital, allergies, lab results, orders for tests, transcribed reports (discharge plans, specialist evaluations, etc), pathology, microbiology, pharmacy and prescriptions, radiology, and more. |
