The Waterloo Wellington LHIN has received and reviewed 88 proposals from local organizations in support of the recently announced Aging at Home Initiative.

Description of Services and Units

The program descriptions for Community Support Services are changing and the 2002/03 Provincial Performance Funding and Allocation Manual is being updated.  More than half of the current CSS providers are early adopters of the new codes and the remaining CSS providers are using the codes from the 2002/03  PFA manual.  Non Service providers or those new to service provision would benefit from reviewing the 2002/03 PFA manual as it provides descriptions and definitions of services.

Background

On August 28th, 2007 the Ministry of Health and Long-Term Care announced an investment of $702 million over three years for an Aging at Home Strategy.  This Strategy is aimed at providing seniors and their families and caregivers with an integrated continuum of community-based services to enable them to stay healthy and live more independently in their homes.  Please click on the link below to learn more about the provincial Aging at Home Strategy.

As a part of the Strategy, the Waterloo Wellington LHIN received $37million and will lead an exciting initiative that will impact the way services are delivered and help provide more equitable access to health care by matching the needs of the local senior population with the appropriate support services.

The Strategy aims to keep seniors healthy by providing a comprehensive mix of services for seniors and their caregivers through traditional services combined with innovative practices.   The new funding is targeted to 2 main areas:

1.  Increasing the overall supply (mix and quantity) of services (e.g. capacity building) that support seniors to stay healthy and live safely at home with independence and dignity, including:

  • Community support services
  • Home care
  • Assistive devices
  • Supportive living
  • Long-term care beds
  • End-of-life care

2.  Leveraging change through innovation, such as new preventative and wellness services and partnerships with non-traditional providers.

The Waterloo Wellington LHIN supports the provincial Aging at Home goals and objectives.  We have and will continue to work with the community to develop an aging at home local plan to provide a suite of services for seniors to enable them to live as independently as possible, for as long as possible, in a safe home of their choice”

The WWLHIN Aging at Home Strategy will:

  • Make investments into community and long term care settings.  The strategy will improve access to care by assessing long term care bed capacity, and by building supportive housing units
  • Enable seniors to remain at home for as long as possible by implementing innovative health service delivery models, and funding more community based services
  • Increase the skills and knowledge of staff in the long term care system to more effectively manage their medically complex needs
  • Enhance innovative health promotion strategies to keep seniors well

Invitation to Participate in the Development of an Aging at Home Service Plan

The Waterloo Wellington Local Health Integration Network (WWLHIN) is inviting pre-proposals for initiatives that support the implementation of the MOHLTC Aging at Home strategy.  The plan will outline the services and innovations that will be put in place to achieve the local AAH goal to enable seniors to live independently. 

Request for Ideas and Innovations for Senior Care in WWLHIN

The WWLHIN is inviting the community to respond with ideas and innovations that target the following populations and target areas:   

Complex Geriatrics

Frail Elderly

Supportive Housing

Seniors Health and Wellness

The Ideas and innovations proposed with be reviewed by the WWLHIN and those ideas/innovations that best align with the Integrated Health Service Plan, the Aging at Home Directional Plan, the needs of the seniors in Waterloo Wellington LHIN and alignment with leading practices and evidenced based case will move forward to a business case request.  Business cases will be reviewed internally and funding will be awarded.   Those business cased approved and accepted will become part of the Aging at Home Service Plan.
