Call for Waterloo Wellington Local Health Integration Network Aging at Home Proposals for Year Two (2009/10)

1. Overview

The Waterloo Wellington Local Health Integration Network (WWLHIN) is issuing its Call for Proposals on August 21, 2008, in support of year two (April 1, 2009-March 31, 2010) of its Aging at Home (AAH) Strategy. The AAH Strategy is a provincial initiative launched in August 2007 by the Ministry of Health and Long-Term Care (MOHLTC). The Strategy represents a $700 million province-wide investment over three years, with $37 million being allocated to Waterloo Wellington. The WWLHIN has identified available funding of $2 million in support of year two proposals.

The WWLHIN requires all interested applicants to complete a Year Two AAH Strategy Proposal Template by September 30, 2008 at 4:00 PM. For the benefit of interested applicants, the following information is found below:


2. Purpose of the WWLHIN Aging at Home Year Two Service Plan

The focus and anticipated outcomes of the WWLHIN AAH strategy directly align with the priority areas of the WWLHIN Integrated Health Service Plan (IHSP), as well as its Annual Service Plan (ASP) (2008/09). The overall goal of the Aging at Home Strategy in the WWLHIN is to offer a suite of services for seniors to enable them to live as independently as possible, for as long as possible, in a safe home of their choice. The WWLHIN, through its communities’ input and environmental scanning activities, has identified three focus areas to accomplish this goal:

  • Services for Frail and Complex Seniors
  • Supportive Housing Services
  • Seniors’ Health and Wellness

The year two Service Plan builds on the investments committed in year one of the Aging at Home Strategy, and the opportunities that are identified in the WWLHIN AAH Directional Plan, released in October 2007. The WWLHIN has also determined that year two proposals dealing with any of the three focus areas must also show how they will sustainably relieve either emergency department (ED) wait-times or alternative level of care (ALC) pressures in Waterloo Wellington. To that end, proposals are expected to include performance measures and outcomes that relate to the relief of ED wait-times or ALC pressures.

Consistent with the approach taken in year one, each proposal should offer enhanced, integrated and higher quality service provision for seniors, and an increase in effective, efficient and better coordinated services for seniors than are currently available.


3. Eligibility: Respondents and Scope

Respondents:

This Call for Proposals is open to all organizations that can fulfill the purpose of the WWLHIN Aging at Home Year Two Service Plan. Health service providers, other service providers, and community, volunteer, multicultural, business, academic, and civic groups are all welcome to submit proposals for solutions in any of the three AAH focus areas. Recipients of funding committed through year one of the AAH Strategy are also welcome to submit proposals.

Scope:

The following areas or requests for funding will not be considered in year two proposals:

  • Single funding requests in excess of the identified available funding of $2 million
  • Planning
  • Purely academic research
  • Capital
  • Large technology infrastructure (information technology [IT] or information management [IM]) projects
  • New long-term care Home beds
  • Proposals authored by single organizations with no collaboration or integration opportunities identified

4. Timelines and Submission Process

November 24, 2008 - The Waterloo Wellington Local Health Integration Network (WWLHIN) continues to make good progress in its assessment of proposals received in response to its Call. Consistent with step nine of its "Approach to Decision-Making in the Context of Priority-Setting" (dated September 12, 2008), the WWLHIN is applying relevant criteria to the remaining eligible proposals. Alignment of Aging at Home funding with other WWLHIN strategies to address Emergency Department wait-times and Alternative Level of Care pressures is a specific criterion currently under consideration. Completing this work properly will lead to the submission of recommendations to the WWLHIN Board of Directors in time for its January 22, 2009, meeting.

5. Support to Applicants: Teleconferences, Background on Aging at Home

To support the creation and submission of complete proposals for September 30, the WWLHIN will host two teleconferences. The first teleconference will occur on Friday, September 5, 2008, from 11:00 AM to Noon. In response to the questions posed or clarification sought during this teleconference, the WWLHIN commits to refreshing its Questions-and-Answers document and reposting it on the WWLHIN website by Wednesday, September 10, 2008.

The second teleconference will occur on Friday, September 12, 2008, from 2:00 to 3:00 PM. Also in response to the questions posed or clarification sought during this second teleconference, or questions received in the following week, the WWLHIN commits to refreshing its Questions-and-Answers document and reposting it on the WWLHIN website by Friday, September 19, 2008. After September 19, 2008, the WWLHIN will decline responding in this manner to additional questions. 6. Evaluative Criteria

The WWLHIN will assess proposals in relation to the following criteria:

  • Alignment with IHSP and the WWLHIN’s vision for an integrated, accessible, population focused, efficient and sustainable health system
  • Achievement of Aging at Home goals, as demonstrated by alignment with the focus areas of
    • Services for Frail and Complex Seniors
    • Supportive Housing Services
    • Seniors’ Health and Wellness
  • Collaborative partnerships with designated lead organizations
  • Embedded performance targets linked to reductions in ED wait-times and ALC pressures
  • Readiness to implement the project by April 1, 2009
  • Organizational capacity, or future likelihood of creating capacity, to deliver proposed services or programs
  • Demonstrated sound financial management
  • Demonstrated evidence of multi-sectoral partnerships and coordination
  • Demonstrated involvement of senior/consumer in the project
  • Geographic and service alignment in the context of current health service provision and implementation of year one investments
  • Equity in the provision of services to recognizable populations

8. Proposal Template

The link to the proposal template, located as a separate document on the WWLHIN website, is noted above.

The template is in “MS Word, Version 2003” format and contains fields that automatically change in size to the appropriate text size and font. Completion of all fields in this template is a requirement for consideration of a proposal.

The WWLHIN will accept proposals by e-mail; the completed proposal template should be a “Word” attachment in the e-mail. A submission is deemed complete by the WWLHIN if all fields of the template are filled-in and the template is submitted by e-mail no later than 4:00 PM on September 30, 2008. Submissions should be sent to Sonal Garach at sonal.garach@lhins.on.ca.

In cases where an organization cannot make an electronic submission of the proposal template, the WWLHIN must be contacted in advance of the submission deadline to make other arrangements (e.g., hard copy) that still meet the deadline.

Late proposals will not be accepted.
